Please provide a brief narrative (3 pages maximum) that includes:

  1. Brief program description and program goals. 
  2. Identification of the targeted population and planned recruitment efforts.
  3. Plans to assess the impact of the program including: 
    1. Quantitative measures used to assess student outcomes (please provide name of assessment, if possible)? 
    2. Qualitative measures used to assess student outcomes (e.g. student surveys, exit interviews, etc.)? 
    3. If applicable, plans to track student progress after the program (e.g. matriculation, etc.)
