Student Athletic Survey

Purpose of the Athletic Survey:

This data collection is being conducted for evaluation, and planning purposes and may be used along with other information to determine whether your institution is effectively accommodating the athletic interests and abilities of its students, including whether to include additional teams.  All responses, as well as any personally identifiable information—your name, address, email and phone number—will be kept confidential.  If you express interest in being contacted, your information will still be kept confidential up to the extent needed to follow-up with you. The combined results of the survey will be shared with the College President and her/his executive staff, the athletic director, coaches, and other staff, as appropriate.

Please answer all applicable questions about your ability, current participation, interest in future participation and experience in the sports below. You do not have to answer every question. If you do not respond to a question, missing responses will be treated as “No Current Participation,” “No Interest in Future Participation,” “No Experience,” or “No Ability,” depending on the question.
